About this template

The most common complaint about therapy is forgetting what you meant to say, then forgetting what was said. A therapy journal fixes both. This template has a short section to fill before a session, with the things that happened this week and the one thing you want to make sure you raise, and a longer section afterwards for what came up, what surprised you and what the therapist asked you to try.

It is for anyone in counseling, psychotherapy or coaching, weekly or less often. Schedule it for the day of your session and the app will prompt you. Over a course of sessions the “what I am taking away” lines become a record of progress that is easy to lose track of otherwise, and the homework prompt keeps the work going between appointments, which is where most of the change happens.

How to use it

  1. Fill in the “before” section the night before or in the waiting room.
  2. Write the “after” section within a few hours, before the details fade.
  3. Keep the homework line concrete: an action, not a theme.
  4. Reread the last three entries before each session. It is the fastest way to pick up where you left off.
